Following qualification as a pharmacist, I gained experience across both emergency care and primary care settings, developing a broad understanding of medicines optimisation and the importance of collaborative working across healthcare environments.
I joined Durham Dales Health Federation in 2021 as a Clinical Pharmacist and have since progressed to an Independent Prescribing Pharmacist before recently being appointed as a Senior Clinical Pharmacist. Working collaboratively across our Primary Care Network (PCN), I support GP practices and the wider multidisciplinary team to optimise prescribing, improve patient outcomes, and deliver safe, evidence-based, person-centred care.
My role includes undertaking structured medication reviews, leading hypertension and lipid management clinics, and supporting the management of patients with long-term conditions. I regularly participate in care home ward rounds and work closely with the local STOMP team to review medicines for people with learning disabilities and other vulnerable patients with complex needs. Through collaborative working and shared decision-making, I strive to ensure prescribing decisions are safe, evidence-based, and tailored to individual needs, enabling patients to make informed choices about their medicines while supporting improved health outcomes and quality of life.
Alongside my clinical responsibilities, I contribute to the development and improvement of pharmacy services across the Federation through reviewing and implementing standard operating procedures (SOPs), promoting clinical governance, and supporting consistent, high-quality practice. I am passionate about fostering a culture of continuous learning and professional development, both for myself and within the wider pharmacy team, to continually strengthen clinical skills and improve patient care.
My journey from apprentice Pharmacy Technician to Senior Clinical Pharmacist has given me a unique perspective on the evolving role of pharmacy within primary care. I remain committed to compassionate, evidence-based practice and working collaboratively with patients and healthcare professionals to deliver meaningful improvements in medicines optimisation and health outcomes for our local communities.
